Abstract: Introduction: the tool production based on knowledge translation, using evidences of caring for children with special healthcare needs (CSHCN), could reduce the gap between knowing and doing. Objective: To produce and evaluate tools of caring for CSHCN with complex care, challenges to family and primary healthcare in Rio de Janeiro city. Method: A methodological study was conducted by knowledge-to-action model of knowledge translation, applying three steps of the creation circle: knowledge inquiry, synthesis, and tools. Firstly, the narratives, selected from data bank of previous research developed by the Research Group, were added to narratives from Facebook. The content analysis were applied and generated three main themes of care: after colostomy; for gastrostomy, for tracheostomy. An integrative literature review was implemented by selecting a best evidences of caring for each one situation in order to constitute a knowledge synthesis. Then, we tailored narratives with evidences for creating a tool knowledge. Results. Three tools were presented in different formats, one comic (after reversal surgery colostomy) and two maps of care (tracheostomy and gastrostomy). The comic included care for CSHCN after reversal surgery of colostomy. The map of caring for CSHCN with tracheostomy, and, with gastrostomy have included contents of health rights and complex care. The scientific language were simplified for common reader understand, and images illustrate an everyday of caring at home. Conclusion: the three steps of knowledge creation help us to find best evidences, tailored narratives and generate a creative tools for common reader to continuous complex care for children.
